Overview
The Board of Nursing is a eleven-member board created by the legislature whose members are appointed by the governor to administer the laws for this profession in the state of Vermont. The board’s mission is public protection. The board does this by: ensuring that applicants are qualified for licensure; setting standards for the profession by proposing statutes and adopting administrative rules; and, with the assistance of Office of Professional Regulation staff, investigating complaints of unprofessional conduct, taking disciplinary action against licensees when necessary to protect the public.
Registered Nursing (RN) Licensure by Examination
Applicants Must Meet the Following Requirements:
- Complete the application.
- Submit the non-refundable application Fee of $90.00. Checks/Money orders are payable to Vermont Secretary of State. Credit/Debit cards are not
- Attach a 2×2 Passport sized photo of head and shoulders taken within the last 6 months other than your driver’s license or passport.
- Submit the Verification of Education Form included within the application This verification of education form must be completed and placed in a sealed envelope by the school. The school can either:
- Send the completed verification form, signed, dated, and affixed with the school stamp/seal along with a copy of official transcripts directly to the Vermont Board of Nursing
OR
- Give the completed verification form, signed, dated, and affixed with the school stamp/seal along with a copy of official transcripts to you so you may attach it unopened to your application.
NOTE: Official transcripts are NOT required If the nursing program was completed in the State of Vermont.
- Submit copy of your passport, driving license, or government issued ID.
- Register with Pearson VUE to take the NCLEX-RN exam. You may use any of the following ways to complete the registration:
- Register online. The registration fee of $200 can be paid via American Express, Visa, or MasterCard. OR
- By mail: You may request the Board of Nursing office to send an NCLEX registration form. Provide essential details in the form and include an appropriate fee of $200.00 in the form of a money order or cashiers check made payable to the NCSBN (National Council of State Boards of Nursing). - When all your essential documents are approved, the Board will inform the testing agency, Pearson VUE, making you eligible to take the licensure exam.
- Pearson VUE will send you an ATT (Authorization to Test) via email or on the address provided by you during the registration process.
- Upon receiving the ATT, schedule the licensure exam.
- Submit the completed application and documents to the following mailing address:

Licensed Practical Nurse (LPN) Licensure by Examination
Applicants Must Meet the Following Requirements:
- Complete the application.

- Attach a 2×2 Passport sized photo of head and shoulders taken within the last 6 months other than your driver’s license or passport.

Applicants Must Meet the Following Requirements:
- Complete application.
- Submit the non-refundable application Fee of $150.00. Checks/Money orders are payable to Vermont Secretary of State. Credit/Debit cards are not
- Provide verification of your original nursing license as well as the nursing license from your most recent state of nursing employment. If those states are members of NURSYS, go to nursys.com and obtain license verification(s) for Vermont. Note: The Quick Confirm Report does not suffice. If those states do not participate in electronic verification through NURSYS, use the additional pages 8 and 9 in the application packet to request verification from those boards.
- Submit copy of your passport, driving license, or government issued ID.
- Attach a 2×2 Passport sized photo of head and shoulders taken within the last 6 months other than your driver’s license or passport.
